Palliative Care

For those who have a terminal illness, to be able to spend the remainder of your days at home, surrounded by your loved ones provides comfort and dignity in circumstances that can otherwise be stressful.

What types of help can you get?

• Symptom control, including medication, education and telephone support.
• Arranging aids to help you live at home.
• Liaison with your GP, Occupational Therapist or Physiotherapist.

Accessing the Service

You may approach be referred by your general practitioner (G.P.) a Needs Assessment and Service Coordination (NASC) agency or through the hospital system.
