Community Nursing

Whether you are requiring some short term support immediately after you leave hospital, or ongoing support over a longer period of time Ali's Nursing Service will take care of you.

Our dedicated team of nurses takes an holistic approach to care, ensuring that you are being looked after in all aspects of your life and where your needs are beyond the scope of the services we are contracted to provide, we will guide you in the right direction for support.

What types of help can you get?

• Assistance with taking medication.
  

Health Checks

• Ongoing support to ensure that your condition is being maintained and that you do not need any additional treatment.
• Ensuring your safety in your home.
• Blood pressure checks.
• Liaison with your GP and other health professionals if required.  
  

Diabetes

• Education and support.
• Blood sugar monitoring.
• Insulin administration.
  

Stomal Support

Wound Care

• Treatment of surgical wounds.
• Chronic ulcers.
• Skin graft/donors.
• Skin lacerations/burns.
• 'Vac' dressings.
  

Urinary catheter care

• Management of indwelling and Supra pubic catheter.
  

These services can help support you to safely remain in your own home where you have a temporary or permanent disability. The services are available seven days a week if required.

Accessing the Service

You may be referred by your general practitioner (G.P.) a Needs Assessment and Service Coordination (NASC) agency or through the hospital system.

Where you have been assessed by any of the agencies above as having a need, public funding is available for the services and you do not have to pay.

Where you do not qualify for public funding, you may also pay personally.
